MAJOR RESPONSIBILITIES/PERFORMANCE OBJECTIVES:Coordinates with various other departments to determine transportation needs and facilitates daily transportation of patients for medical, judicial, or programmatic needs. Conducts general inspections of the vehicles before/after each transport to insure security and proper functioning/status of equipment. Reports any condition not in keeping with expected status to the Facility Manager. Helps provide supervision of patients during transports and upholds the behavior management plans of those patients during transports. Participates in an On-Call- Driver rotation to provide transportation for emergency events during weekends. DUTIES: 1. Provides transportation to residential, partial hospitalization and day program.2. Checks status of vehicles before and after transports to ensure proper operation of equipment and completes relevant transportation documents/logs.3. Reports any maintenance needs to Transportation Manager.4. Helps provide supervision of patients on transports, assuring that safety measures and policies are observed by all riders, such as use of child safety seats for designated children, use of seatbelts by all riders, assuring no eating or smoking in vehicle.5. Attends staff meetings as needed.6. Maintain core competency.7. Ensure immediate reporting of injuries and/or unsafe conditions to supervisors, to include appropriate documentation.8.
